Drug Insurance
Choice between three
Carefirst BlueCross/Blue Shield health insurance
plans. All plan are excellent, offering comprehensive
medical coverage.
BlueChoice
HMO - BlueChoice HMO provides the cost
savings of an HMO and the flexibility to choose
a primary care physician (PCP) from a network
of over 2,600 PCP locations, specialty care
from over 14,000 locations and hospital care
from 55 area hospitals. There is an $18.00 per
pay period individual employee contribution
for this plan.
BlueChoice
Opt-Out Plus - BlueChoice Opt-Out Plus
is a point of service (POS) that offers the
cost savings of an HMO with the freedom of choice
and out-of-pocket savings received through traditional
CareFirst BlueCross Blue Shield coverage. There
is a $20.60 per pay period individual employee
contribution for this plan.
BluePreferred
- BluePreferred is a preferred provider organization
(PPO) health care plan that offers the flexibility
to choose your doctor or hospital and control
your health care costs. You save money when
you obtain your health care from our large Preferred
Provider network, and you can self-refer to
any physician or provider in the network. There
is a $22.35 per pay period individual employee
contribution for this plan.
Carefirst
Dental Insurance
- See a dentist within the large network of
participating dentists or chose a dentist outside
of the network. Included with your health
insurance plan.
Qualified dependents and
spouses may participate in group health and dental

(k) Plan
Great-West Life & Annuity
A 401(k) is an ideal savings
plan that allows you to invest money, have tax
breaks and is a very important tool to help you
achieve your financial goals. This benefit provides
the employee the opportunity to invest money in
29 diverse funds with tax-free dollars. Based
on years of service employer matching is 5% of
yearly contributions.

Great-West Life &
Annuity
This benefit provides the
employee the opportunity to set up an individual
account into which tax-free dollars can be deposited
through payroll deduction. The account(s) are

Assistance and Reimbursement
Ultra Tech reimburses employees
for tuition charges incurred for pre-approved
job related courses taken at an accredited college,
university, professional or technical school.

Referral Bonus Program
Employee receives a $500
bonus if his/her referral is hired. The bonus
is paid if both employees are onboard at the end
of the referred employee’s 90-day probationary
period. The bonus is subject to applicable tax
laws.

Unum
Life, Accidental Death and Dismemberment Insurance
Plan
Life policy is equal to one
times the employee’s annual salary. AD&D policy
is equal to one times the life policy. For persons
70 or older, the life policy is equal to 67 percent
of the employee’s annual salary and the AD&D policy
is equal to one times life policy.
National Capital Insurance
A $10,000 supplemental life
and AD&D insurance provided at no cost to employee.
Unum
Short Term Disability
Short-term Disability provides
60 percent protection of employee’s weekly earnings
up to $2500 per week. Benefits are payable up
to 13 weeks. Benefits begin immediately for injuries
and day eight for illness.
Unum
Long Term Disability
Long-term Disability provides
60 percent of employee’s monthly earnings up to
$5,000 per month should employee become totally
or partially disabled and is unable to return
to work after 13 weeks. Benefits are payable up
to age 65, provided that employee remains totally
or partially disabled. Payments can be offset
by Social Security payments.
Unum
Employee Assistance Program
This program provides employees
assistance with personal issues. UNUM's LifeBalance
Program provides information and consultation
on a wide range of topics/issues including legal
and financial issues, older adult and parenting
resources, personal and work-related problems,
and other personal issues.
